1. $75.00 fee – checks payable to GHLL

2. The original Birth Certificate issued by the county not the hospital (No footprints)

3. A copy of parent’s WDL with current address

4. A copy of parent’s phone bill or light bill with current address

5. A copy of homeowners or automobile insurance with current address

6. A completed medical release form; NOTORIZED

Without these items and completed forms your child

will not be eligible to play on an All-Star team.

Birth Certificates will be returned at the end of the All-Star Season.
